Nurse Practitioner Primary Care - Westchester
Nurse Practitioner will be providing primary care directly in patients' homes throughout the assigned service area. Field-based position focused primarily on older and medically complex patients who have difficulty accessing traditional office-based care. The NP manages patients longitudinally, including comprehensive assessments, chronic disease management, medication management, treatment planning, follow-up and coordination with the broader care team
DUTIES: Provide primary care to high-acuity patients in their homes. Perform comprehensive patient assessments. Diagnose and treat medical conditions. Manage chronic diseases. Manage medications. Develop and update individualized treatment plans. Provide ongoing/longitudinal patient care. Coordinate care with physicians, nurses, caregivers and other providers. Identify changes in patient condition and intervene appropriately. Support quality/value-based care outcomes. Complete timely clinical documentation. Maintain patient confidentiality. Participate in required meetings, training and professional development. Maintain required licenses and certifications.
QUALIFICATIONS: FNP/ANP 2 plus years as a NP doing Primary care, with home care experience .

The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs. Active NP license in New York. Board Certification. Federal DEA license or eligibility. Active CPR/BLS certification. Valid driver's license. Good driving record. Comfortable traveling to patients' homes. Comfortable treating medically complex/high-acuity patients. Strong clinical and documentation skills.
